Description
The OASIS tool is often viewed as another required document to be completed by healthcare professionals. However, with knowledge and skill, the OASIS can be an important tool in the process of patient assessment data collection, evidence-based care planning, and improving client outcomes.

This module provides the home care professional with knowledge of appropriate assessment information to gather at the time of discharge or transfer of the client and appropriately apply it to OASIS items.
Delivery

This is a self-paced module designed to support independent learning. It includes optional knowledge checks and reflection activities to reinforce key concepts throughout the module. To earn a Certificate of Completion, participants must do the following: • Achieve a score of 80% or higher on the Final Exam (Unlimited attempts allowed; reviewing course materials between attempts is encouraged) • Complete the Post-Module Evaluation All course components—including content review, Final Exam, and Survey

Objectives
  • Refer to guidance regarding the correct use of “Death at Home”
  • Follow agency policies regarding the handoff of a reconciled medication list
  • Conduct a skin assessment as part of an agency discharge visit

This course is designed to meet the continuing education requirements of health care professionals such as Registered Nurses, Physical Therapists, Speech Therapists, and Occupational Therapists.

Cindy Krafft - PT MS HCS-O

Cindy Krafft PT, MS, HCS-O is an owner of K&K Health Care Solutions based in Florida. She brings 30 years of home health expertise that ranges from direct patient care to operational / management issues as well as a passion for understanding regulations. Years spent in the direct patient environment solidified that this was the optimal setting for the provision of effective and efficient interdisciplinary care as it focuses on functionality and patient centered care delivery. Recognition that the providing care in the home environment presents both unique opportunities and challenges is clearly evident in both training and consultation activities. For the past nearly 20 years, Cindy has been a nationally recognized educator in the areas of documentation, regulation, therapy utilization and OASIS. She has and currently serves on multiple Technical Expert Panels with CMS Contractors working on clinical and payment reforms and bundled payment care initiatives. Her focus is on providing the knowledge and tools critical to the post-acute continuum of care to operationalize external requirements while keeping the driver of care where it needs to be – the needs of each patient being cared for.
