Introduction to Home Care for Therapists
Learning OutcomesAs a result of participating in this learning activity the learner will be able to:
- Demonstrate knowledge of basic home care background, principles and practice by passing the post-test with a score of > 80% following the on-line learning activity.
This is a self-paced course with knowledge check activities. These non-graded activities allow you to check your understanding of the lesson objectives before proceeding on in the course. At the end of the course, you will be required to take the final exam. You must score 80% or higher to pass the course and complete the evaluation in order to obtain a certificate of completion.
- Identify common home care components including rules, regulations, payer sources and terms
- Describe the various disciplines/roles in home care, their duties and the therapist’s role in supervision
- Outline the referral, intake, admission process in establishing the plan of care
- Distinguish basic OASIS assessment and documentation guidelines and components for Medicare clients in home care
- Explain the importance of the therapy goals and plan of care in prioritizing effective treatment
- Identify base discharge criteria for home care clients

Cindy Krafft - PT MS HCS-O
Cindy Krafft PT, MS, HCS-O is an owner of K&K Health Care Solutions based in Florida. She brings 30 years of home health expertise that ranges from direct patient care to operational / management issues as well as a passion for understanding regulations. Years spent in the direct patient environment solidified that this was the optimal setting for the provision of effective and efficient interdisciplinary care as it focuses on functionality and patient centered care delivery. Recognition that the providing care in the home environment presents both unique opportunities and challenges is clearly evident in both training and consultation activities. For the past nearly 20 years, Cindy has been a nationally recognized educator in the areas of documentation, regulation, therapy utilization and OASIS. She has and currently serves on multiple Technical Expert Panels with CMS Contractors working on clinical and payment reforms and bundled payment care initiatives. Her focus is on providing the knowledge and tools critical to the post-acute continuum of care to operationalize external requirements while keeping the driver of care where it needs to be – the needs of each patient being cared for.
Iona Kaye (Katie) Barton - PT Ed. D MEPD
Katie received both her Bachelors of Science in Physical Therapy and her Masters of Education in Professional Development from the University of Wisconsin - La Crosse. She received her Doctorate in Education Leadership from Saint Mary's University in Minneapolis, MN. Katie was an adjunct instructor for the physical therapy program at the University of Wisconsin - La Crosse for 17 years. She has a varied background of healthcare experience including birth-3, hospital based pediatrics, school based PT, SNF PT, and most recently, 6+ years as a physical therapist for a hospital based home care and hospice agency.
Michelle Olson - M.S.P.T.
Michelle received her Bachelors of Science in Biology from the University of Wisconsin - La Crosse and her Masters of Science in Physical Therapy from Carroll College. She has been practicing as a physical therapist for a hospital based home care and hospice agency for the past 8 years and has had experience in sports medicine and pediatric home and clinic based care. Michelle has a special interest in physical therapist education and has also been employed as a lab assistant/associate lecturer at the University of Wisconsin - La Crosse for the past 5 years. Michelle is an APTA certified clinical instructor and has been taking students into the home care setting for the past 6 years.
